BREAST AUGMENTATION TECHNIQUES
Implant Sizing
Proper implant sizing is paramount for creating a realistic breast appearance. Dr. Schwartz uses a measurement system that he developed to determine your perfect size. We don’t go over. By only placing implants that fit you perfectly, we avoid using implants that are too large and more prone to displacement. Implants that are too big for your physicality usually come with a longer recovery and a higher risk of losing nipple sensation.
Implant Options
- Silicone implants are comprised of a soft silicone shell and filled with a soft silicone gel. Many patients prefer silicone implants because they better simulate the look and feel of natural breast tissue.
- “Gummy bear,” or cohesive gel implants are called form-stable since they are thicker, maintain their shape, and can be anatomical. These implants are beneficial for patients who desire more natural-looking breasts since they are smaller at the top and larger at the bottom. Dr. Schwartz is one of the highest-volume users of gummy bear implants in the US.
- Saline implants are comprised of a silicone shell filled with a saltwater solution. These implants can be custom-filled during your surgery to ensure an appropriate size. However, they are more prone to visible rippling or wrinkles in patients who have thinner breast tissue. Saline implants are more likely to displace laterally over time.
Incision Location
- The inframammary fold (IMF) incision hides the potential scar in the breast crease and is more likely to preserve the nerve functions of the breast. This technique lowers the risk of introducing bacteria into the implant pocket, minimizing your chance of developing capsular contracture.
- While less commonly used, periareolar incisions can be discussed during your consultation to see if they will better help you achieve the results you desire.
Implant Placement
- Subglandular placement places the implants below the glandular tissue but above the pectoral muscles. This is usually associated with a less painful recovery and does not cause implant movement during muscle contractions. However, upper pole edges are more likely to be visible in patients with thinner breast tissue, and there is an increased risk of capsular contracture.
- Subpectoral placement, or dual plane, places the implants below both the glandular tissue and the pectoral muscles. This creates a more natural slope of the upper pole and can help mask the edges of the implants. It’s often a better choice for thinner patients.

Your consultation with Dr. Schwartz is the most important step in your breast journey. During your consultation, VECTRA® 3D imaging provides Dr. Schwartz with precise, detailed information about your breast size and tissue distribution. Implant simulators are used during your consultation to help you better visualize your potential results. Additionally, you and Dr. Schwartz will discuss your aesthetic goals and review your medical history to ensure that you are healthy and a good candidate for treatment.
During surgery, Dr. Schwartz uses a gentle type of anesthesia known as TIVA (total intravenous anesthesia). Using only IV medications, you will be asleep but more like when you sleep at nighttime. This adequate state of anesthesia does not require a breathing tube or anesthetic gases; you are essentially asleep for the duration of the procedure but breathing on your own.
TIVA medications wear off quickly and are eliminated from your body within hours of surgery. This results in a shorter time in the recovery room as well as an overall smoother experience. TIVA is also associated with less or sometimes no nausea or vomiting; in fact, the IV medication has an anti-nausea effect. You will likely feel good enough to go out for dinner the night after your surgery.
Dr. Jaime Schwartz has developed a combination of techniques called Rapid Recovery™ that accelerates the recovery process and helps minimize discomfort after breast augmentation. In addition to the anesthesia technique (TIVA), Dr. Schwartz may inject BOTOX® into the muscles of the chest prior to surgery and use EXPAREL® (a long-acting local anesthetic) in the muscles during surgery to produce postsurgical analgesia. Most of our patients take only TYLENOL® after their breast augmentation procedure.
Typically, patients return to work after one week and may be able to resume some exercise after four weeks and full exercise at six weeks. We prescribe medical prophylaxis for our patients, which is medication to help prevent capsular contracture if indicated.

While there’s always a risk of losing nipple sensation, Dr. Schwartz, when performing this procedure through an inframammary fold incision, has had great success in keeping all nipple sensation.
Yes. Implants that aren’t proportional to your body size can create an imbalanced appearance. Studies have also shown that implants over 400 cc can result in cervical spinal changes.
Implants alone won’t improve sagging breasts. Dr. Schwartz recommends that patients with breast ptosis combine their implants with his LipoLift™ procedure. The FDA recommends replacing implants after 15 years. After this time, implants are more likely to degrade and result in ruptures, leaking, or malposition.
Dr. Schwartz uses an inframammary incision to preserve the nerve functions of the breasts. Most women don’t have issues nursing with implants. However, you may experience difficulties with implants that are too large and place pressure on the ducts.
No. There is no study that supports the idea that breast implants cause cancer. However, anatomic gummy bear implants have been linked to ALCL. However, out of the millions of implants placed, only 400 cases of ALCL have been associated. Additionally, Dr. Schwartz uses Mentor® implants, which have been linked to only four cases because they have less aggressive texturing.
